Use this private working sheet to organize a discussion with your prior acquirer and any prospective provider. Print it, or save a blank PDF and keep your notes securely.
This worksheet does not establish MATCH status, obtain a removal or determine processing eligibility. Nothing you write on your printed copy is submitted to NUMUS. Do not record full card numbers, security codes, passwords or Social Security numbers here.
01 Reference details
02 Questions for the prior acquirer
Was my business reported to MATCH, or was the account only declined or terminated?
If a listing exists, which entity, reason code, date and reporting acquirer are associated with it?
What underlying account records explain the issue, and which can you provide?
If information appears inaccurate, who handles the investigation and what evidence should I submit?
What is the status of held funds, reserves, refunds, disputes and other outstanding obligations?
Keep the listing question separate from the held-funds question. A response on one does not resolve the other.
03 What was confirmed / what remains unknown
Record the date, who provided the answer and which document supports it.

Merchant review worksheet 02 / Prepare the conversation
Evidence before promises.
Organize the packet. Question the offer.
A provider may require different or additional records. Ask for its exact requirements and an approved secure delivery method before sending personal or business documents.

05 Questions before accepting an offer
Have I disclosed the termination and any confirmed listing accurately?
Will the acquiring bank consider these circumstances? Who makes the decision?
Which fees are due before a decision, and when are they refundable?
Which pricing, minimums and account fees are documented in writing?
What reserve, payout timing and volume limits would apply?
What products, billing methods or transactions would be restricted?
What contract term, termination fees and security obligations would apply?
Is anyone promising guaranteed approval, automatic removal or a way to hide the history?
06 Agreed next step / owner / date
A new business name, concealed ownership or an inaccurate application is not a legitimate shortcut. A review, a quote and a complete packet are not approval. Read the current Mastercard rules and get the acquirer’s requirements for your specific situation.
